Applicant's Agreement

Payment Agreement: I/We agree to make payments in accordance with the terms set by David Health International and authorize them to charge my/our company credit card for any outstanding debts from purchases made. I/We confirm that the information provided is correct and I/we have the authority to bind my/our business to this agreement. I/We agree to Cash on Delivery (C.O.D.) or Payment in Advance (PMT. IN ADV.) terms for orders until credit approval is granted.
